Palliative care or end of life support as it is best known, is probably the hardest period to go through for service users and their loved ones. Being at home as you or a family member passes is a feeling and an emotion that most people cherish.

There are so many different types of support that can assist you in this period, giving you or a family member the ability to spend time with that special person in their own environment is essential to their comfort.

Having a care plan that is tailored to your needs helps make that difficult period as supported as physically possible. To make your service at home as comfortable as possible you can incorporate as many services as required from the list below:

  • Personal care – supporting to have a bath or a shower
  • Toileting- support with using the toilet whether it is changing continence pads or using a commode
  • Mobility support- help with standing or sitting and where required use of hoists and stand aids
  • Bed position- Helping you to move position in bed
  • Administering or prompting medication
  • Support with pain management
  • Changing of bedding and housekeeping duties
  • Preparation of meals and snacks to taste and dietary requirements
  • Complex care such as changing catheter bags, stoma support and PEG feeding
  • Ongoing companionship, emotional and physical support

As a team we aim to make your return home and your last days as comfortable and supported as possible. All care professionals are trained and mentored in to their roles for that effective quality of care. All staff supplied to any service user who is home to pass will only receive experienced staff members and where possible the same group of care professionals.
